Recording

Keeping (and looking at) records is an important way by which we can learn from any accidents or near misses. This recording will fall into three amin categories:

Serious injuries - there are specific types of injusry which have, by law, to be reported to the HSE. Things such as broken bones, loss of consciousness from a head injury etc.  Details of these and how to report them can be found on the HSE website here - RIDDOR.

Minor injuries - these are the normal sort of injuries that happen in schools: cuts, burns, scalds etc. These do not need to be reported to any outside agency but there does need to be a record kept of them.

Near misses - in many ways, these are the most important. If you record and act on an incident which could in other circumstances have caused harm to someone then you prevent that harm from happening. Near misses should always be recorded and should be discussed regularly at departmental meetings.
